Next Steps

1. Please download and print the consent form form here - Click here to download (Please download the form to your computer and use pdf viewer, opening the form in browser may not work)
2. Fill Section 2 with your Name, Date of Birth, Date and Signature as appears in CIC Application.(Only original handwritten signatures signed in blue ink will be accepted.)
3. If spouse or common-law partner is included in your application, Fill Section 2.1 with your spouse's or common-law partner's Name, Date of Birth, Date, Signature and relationship as appears in CIC Application.(Only original handwritten signatures signed in blue ink will be accepted.)
4. If kids are included in your application, Fill Section 2.2, 2.3 with your children Name, Date of Birth, Date, Signature and relationship as appears in CIC Application.
5. Scan the signed consent form (pdf file maximum size of file is 2 MB ) and email to us support@applyGCMS.com.
6. Please mention order name - (GCMS or GCMSPlus or CBSA or CSIS) in your email (mandatory for CBSA/CSIS orders).
7. Email should contain all below information of primary applicant only
   1. Order Name(GCMS/GCMSPlus/CBSA/CSIS):
   2. emailID(used in payment):
   3. Family Name:
   4. Given Name:
   5. Date of Birth (YYYY-MM-DD):

*Note: Please make sure there are no strikings or corrections in the consent form. Please leave Section 1 empty. If you do not have a family name, please write your given name in the family name field and leave given name empty.
